We recommend that you arrive 10-15 minutes prior to your scheduled appointment to ensure that you have enough time to complete any necessary paperwork. In order for your visit with us to run smoothly and efficiently, we recommend that you bring the following items with you on your first visit:
Your photo ID and insurance card
Please bring the following pertinent medical records to your first appointment:
- Imaging studies (including x-rays, MRIs, or CT scans)
- Radiology reports
- Prior operative reports,
- Any other information you feel our physicians may find helpful in their assessment of your condition.
Please bring a list of all medications that you are currently taking, along with the dosage and frequency with which you take the medication. Please include supplements and over the counter medicines that you take on a regular basis.
Please wear clothes that allow you to show the affected body part easily. For women with an upper extremity injury, please bring or wear a sports bra or tank top without underwires (these will not interfere with x-rays). Those with a lower extremity injury should wear or bring shorts.
